Demographics Changes
For Providers contracted directly with Lucet (Florida Blue, BCBS of Alabama, BCBS of Arkansas, BCBS of Kansas City, and SCAN Health) please make changes or submit a support case within the Provider Portal at
Login (lucethealth.com).
For any facility or provider that is contracted directly with the local Health Plan, you must contact the Health Plan directly to initiate any demographic changes.
Out of network providers and facilities need to submit a Provider Information for Authorization form.
